1. Acknowledgment of risk. I am voluntarily participating in activities at Golf On, Mississauga, Ontario, including golf simulator bays, clubs and equipment, VIP and party rooms, and the premises (the Activities). I understand the Activities involve inherent risks, including injury from swinging clubs or equipment, injury from simulator equipment, slips, trips and falls, injury or impairment related to alcohol, and property damage or loss.
2. Assumption of risk. I voluntarily assume full responsibility for all risks of injury, illness, death or property damage arising from my participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.
3. Release of liability. On behalf of myself, my heirs, executors and assigns, I release and discharge Golf On, its owner(s), employees and agents from all claims arising from my participation, including claims of negligence, to the fullest extent permitted under the laws of Ontario.
4. Alcohol service. I must be 19 or older to be served or consume alcohol. Golf On may refuse service at its discretion. I am responsible for my own consumption and conduct, and I will not take part in physical activities while visibly impaired.
5. Minors. If the participant is under 18, a parent or legal guardian signs on the minor's behalf, accepts responsibility for supervision, and agrees to all terms of this waiver, including the release in section 3.
6. Photo and video release. I consent to being photographed or filmed on the premises and grant Golf On a royalty-free licence to use the images for promotional purposes. I may withdraw consent for future use by contacting Golf On.
7. Governing law. This agreement is governed by the laws of Ontario and the federal laws of Canada applicable there.
I confirm that I have read and understood this waiver and that I sign it voluntarily.
